<p class="ql-block">　　<b style="font-size:20px;">國家精準康復救助項目是中國殘聯為加強殘疾人康復工作，普遍滿足殘疾人基本康復服務需求，使貧困兒童得到康復而實施的意向殘疾兒童康復救助項目。洛寧縣婦幼保健院是殘疾兒童康復救助項目實施的定點機構，2023年我院繼續(xù)負責國家精準康復救助項目?，F將有關事項公告如下：</b></p> 救助對象 <p class="ql-block">　　<b style="color:rgb(255, 138, 0); font-size:20px;">符合條件的0一14歲視力、肢體、智力等殘疾兒童和孤獨癥兒童。</b></p> 救助條件 <p class="ql-block">　　<b style="font-size:20px; color:rgb(1, 1, 1);">1.0一6歲具有洛陽市戶籍或暫住證，且持有《中華人民共和國殘疾人證》或醫(yī)療診斷證明；7一14歲具有洛陽市戶籍且持有《中華人民共和國殘疾人證》。</b></p><p class="ql-block"><b style="font-size:20px; color:rgb(1, 1, 1);"> 2.視力、肢體、智力殘疾兒童和孤獨癥兒童，有康復需求。</b></p> 救助標準 <p class="ql-block"><b style="color:rgb(176, 79, 187); font-size:20px;">　1. 監(jiān)護人有康復意愿，能夠按照項目相關要求配合做好康復訓練。 </b></p><p class="ql-block"><b style="color:rgb(176, 79, 187); font-size:20px;"> 2.視力殘疾（低視力）兒童平均補助標準為1000元/人/年；</b></p><p class="ql-block"><b style="color:rgb(176, 79, 187); font-size:20px;"> 3.0一6歲肢體、智力殘疾兒童和孤獨癥兒童為16000元/人/年；</b></p><p class="ql-block"><b style="color:rgb(176, 79, 187); font-size:20px;"> 4.7一14歲肢體、智力殘疾兒童和孤獨癥兒童為12000元/人/年。</b></p> 需提交材料 <p class="ql-block"><b style="color:rgb(57, 181, 74); font-size:20px;">　　1.殘疾證或診斷證明；</b></p><p class="ql-block"><b style="color:rgb(57, 181, 74); font-size:20px;"> 2.戶口本原件；</b></p><p class="ql-block"><b style="color:rgb(57, 181, 74); font-size:20px;"> 3.監(jiān)護人身份證原件；</b></p><p class="ql-block"><b style="color:rgb(57, 181, 74); font-size:20px;"> 4.兒童近期2寸照片3張；</b></p><p class="ql-block"><b style="color:rgb(57, 181, 74); font-size:20px;"> 5.攜救助兒童以往的住院或門診病歷、診斷證明、輔助檢查等相關資料。</b></p> 家長及監(jiān)護人的責任 <p class="ql-block"><b style="color:rgb(1, 1, 1); font-size:20px;">　　1.提供項目所需完整材料；</b></p><p class="ql-block"><b style="color:rgb(1, 1, 1); font-size:20px;"> 2.堅持康復訓練（視力康復訓練不少于2個月，其他康復訓練不少于10個月）；</b></p><p class="ql-block"><b style="color:rgb(1, 1, 1); font-size:20px;"> 3.康復項目救助與城鄉(xiāng)居民醫(yī)保報銷不相沖突，為醫(yī)保的補充；</b></p><p class="ql-block"><b style="color:rgb(1, 1, 1); font-size:20px;"> 4.配合完善檔案材料所需的簽字；</b></p><p class="ql-block"><b style="color:rgb(1, 1, 1); font-size:20px;"> 5.監(jiān)護到位，保證患兒在院訓練期間的安全。 </b></p> 登記時間 <p class="ql-block">　　<b style="color:rgb(57, 181, 74); font-size:20px;">即日起0-14歲有康復需求的兒童在監(jiān)護人的陪同下帶戶口簿、殘疾人證原件，到洛寧縣婦幼保健院進行報名、評估，根據報名及評估情況，將符合項目救助的名單上報縣、市殘聯批準（招滿名額為止）。</b></p> <p class="ql-block"><b style="font-size:20px; color:rgb(255, 138, 0);">　　咨詢電話:</b><b style="font-size:20px; color:rgb(1, 1, 1);">0379-66269562</b></p><p class="ql-block"><b style="font-size:20px; color:rgb(1, 1, 1);"> 13525477445</b><b style="font-size:20px; color:rgb(255, 138, 0);"> </b></p><p class="ql-block"><b style="font-size:20px; color:rgb(255, 138, 0);"> 咨詢地點:</b><b style="font-size:20px; color:rgb(1, 1, 1);">洛寧縣婦幼保健院住院部八樓或門診樓二樓（民族路與洛浦路交叉口）</b></p>
